To existing and potential provider partners, please note that ForeverCare recently notified DHS it will not proceed with Phase 2 of the PASSE program. We will continue our Care Coordination obligations under Phase 1 through February 28th. Both DHS and ForeverCare are communicating with existing members, and the combined PASSE leadership teams are meeting with DHS to discuss further transition details. It is our understanding that members will begin transitioning to the other PASSEs in mid-February.
Due to a business decision, Gateway Health and ForeverCare elected not to participate in the second phase of the program.
